First, because it's harder to identify non related high risk couples, while related couples are almost always high risk.

Second, ppl were raised with the idea that incest of any kind is wrong, and so they think so without any thought of their own, or actually without any sort of reason in particular.

I'd say, as long as it's fully consensual, it's fine.
