There needs to be some statistics kept on how many people an account reports. If someone has more than 15% report rate I'd think that is an issue as well. I can't imagine running into 15 reportable people in 100 games.

People who spam report have their report weight drastically diminished.

Unless you're proposing that Riot hires a few hundred thousand people to monitor every single game in real time then the system needs reports. And with that there will always be false reports. As stated before there are systems in place to make them less than an issue.
