Another big part of Executioner's Calling's appeal is that it is a LOT cheaper than Thirster.
I'd think the Passive I suggested would stack additively with the Active. If it was multiplied and not simply added, it would wreck health regain way too much. Add in some Attack Speed and it'd be a fun new item that would be an alternative to Thirster that people wouldn't get pissy at you for buying.
