Was the skin code inside the bag? Because mine, and my friends did not contain a code.

It's not in the box with the trophy, but it's in the actual bag itself. Check that it didn't fall out while you were pulling the box out, mine did.

Pulled the box with the statues out, and the code for the icon and skin was in a small space on the box. The code is made like the riot nasus and graves cards, and they're pretty light, so make sure they didn't fly off or something.

Yeah I literally checked everywhere, lifted the hard bottom part of the bag... no dice.

Same here. My swag bag (or under the statue box) did not have a code.
