You should have a reform card in your email. If you didn't see one, check your spam folder. In order for us to help you, you would need to share the reform card link with us so we can look it over and give you advice on where you're going wrong.

Also, there are several causes of bans beyond the Tribunal system. It's entirely possible it could be for one of those other things as well.
