check your email, you should have a reform card there. This is a permaban, and I imagine you've been banned in the past.

Go here to contact Support. They may or may not be allowed to tell you the email address associated with your account. At the very least, they should be able to show you the reform card explaining your ban.

Permanent bans are almost never removed, so don't get your hopes up. Players only get a permaban if they continue to break the rules again and again after being bans multiple times. Any unlocked skins, champions, or unspent RP are gone and Riot will not refund them or transfer them.
