^ It only happens if someone leaves. So if no one left your game it will start obviously. But instead of going back to the lobby when someone leaves it gives you the 'Active game not found' message.

Yea just happened to me now. Just installed the patch, joined a custom game and when the countdown finished it came up with the message.

Its been happening less frequently though - like 1 in every 2 games instead of like 3 in every 4

It's on ARAM games because people frequently leave ARAMs.
Notice how you never get returned to team select anymore...

Still have the problem i can't really describe it but, it seems to happen when someone left and when you create a custom game if you play alone it's running fine but when i join a game Aram or something like that i'm having this issue.

Yes, it just happened to me in a game. Seems like it's happening more often but that might just be coincidence.
