I suggest reading the information available regarding the Tribunal. It takes several reports in order to create a Tribunal case. The Tribunal pulls a random sampling of the games you have been reported in. It only takes one game out of the sampling in order to be punished.
Judging from the information provided by Riot in terms of how the Tribunal works, this means you did not slip up once in one game. It means you slipped up once or more in multiple games. As you have stated you have been banned three times before, you have certainly had more than your fair share of "one" slip ups. Still, I personally only judge based on the sampling provided, and that one game out of the sampling is enough for me, and others, to hit punish.
You may find the punishment "harsh," but your punishments are increasing in severity because you are a repeat offender. If you really do not want to be banned again or permabanned, I highly suggest taking responsibility for your actions and strive to improve. Blaming the system or others for your behavior will not help you. :\
It's frustrating, I get that, but we're all responsible for our own behavior.
