Blue Icons and Square pings?

Hey,
I recently played a game on the NA server and found out that when someone gets a kill it wont show their champion's icon, only blue. Also the warning pings are square and blue,
How can I fix this?
I've already tried the repair button,

This has happened to me before the last patch. I honestly don't know what is causing it, but it seemed to just happen every few games or so. Kinda gets annoying, but my problem was fixed with the Vi patch.
