Not necessarily. The spin usually lasts long enough to stretch between last-hits. Also, you don't HAVE to last-hit the minions as soon as you're able to kill them. For instance, if you can deal 100 damage with an auto-attack, you don't HAVE to attack the minions when they reach 100HP. You can wait till they're at ~20 HP and not push your lane any harder than any other ADC.
If your spin is about to run out, and there isn't a minion ready to last-hit, then I don't think it's a huge deal to just let it run out and hit Q again. It only costs 45 mana, and you can probably keep it spinning long enough for that to regen on it's own. Alternately, you can hit an enemy champ, or the enemy tower if the opportunity presents itself. Or, you can just risk it and get an extra auto-attack on a lane minion before the last-hit. One hit worth of damage every now and then probably isn't going to hurt you too much anyway.
