Often a dodge happens because the PVP.net client crashes, loses connection, etc. Its not like the player is intentionally queue dodging in these scenarios.
This happened to me during my first Best of 5 match to get to gold, this was pretty upsetting as I believe it cancels the Best of 5 series until you win another match and get 100 LP again.
