Probably just a generic survey RIOT's marketing department sends out on random topics to keep themselves employed.

Based on how RIOT has let this mode die due to Kass abuse, I'm pretty sure they are not going to be fixing anything.

It really speaks volumes how disillusioned the dominion player base is when they are all immediately convinced that OP is lying. I've heard about Riot giving out surveys before, it's not that hard to believe.

Sorry guys, I can't give you screenshots because since I have now completed the survey I am unable to get back into it

The screenshots posted by Golly are correct, though there were more questions.

The only screenshot I can give you is the email survey that was sent to me:

The account is my EUW account, and if you check lolking, I have played close to 500 dominion games (about a 50/50 win/loss ratio) but haven't played any games on that account recently.
