I believe the steps are as follows:

1. Pick Riven and Lee Sin against each other
2. Have Riven use her Q through all 3 iterations
3. During the animation of Riven's third Q, have Lee Sin ultimate her
4. Riven should now be disabled, including summoners and recall

5. Riven's spells are re-enabled upon death

the same problem happened top em today on a ranked game, i re log, restart and re log again, but it didn't fix the problem, i was unable to use my summoner spells, my recall, or any of my skills, i even lvled up, and put a point on my "R" and didn't fix the problem, and the same, lee sin, against a riven XD, but i don't died, so i dunno if that fixed the problem

to solve this bug you can lee kick you again
