So in champion select and if I happen to click a champion too late before the turn is over or something. And I got shown with this window and I can't get out of it, it just shows the window again over and over unless I close the client, meaning dodging and I'd take the penalty for it. My friend and I both have this bug continuously happening.

this happened to me too, stuck with wrong masteries and runes. similar issue happens try and end queue right before you are accpeted.
