Any reason why this isn't an upgrade of Malady? It seems like the ideal item for on-hit champs, but one of the reasons on-hit falls flat is it has no scaling other than reducing the enemy's MR/getting some mpen, or having access to really high AS.
Most on-hitters have to pack a little (or a lot) of tankiness on the side because they have to stick on a target longer to do their damage, so committing another slot to this item might be rough on them.
