I play ADC, and only play with premades, so I'm ADC 98% of the time. I really don't need to deal with item actives (aside from the wards I buy, and even then, I prefer not having it smartcasted).

I don't run push to talk for anything. I play in quiet areas and can adjust the sensitivity well enough to not annoy others. Doesn't having it on 1 strain your hand (if you talk a lot)? You could move it to 2 or 5 or 8 for a more comfortable hand position.
I can type "re" without moving my left hand (changed keybind for "enter" and "shift + enter").
I don't mind moving my right hand for the other ones. I can see how that would be a convenience though. Again, 98% of the time, I'm in a premade on Skype. No need to type really.
Now, I'm not saying that a mouse with extra buttons isn't cool, but buying a Naga (or anything comparable) just to play LoL is a waste of money, in my opinion. If I were the OP, I would go for a Razer Deathadder or similar.
